Disciplinary Action Letter Writer

This letter constitutes a formal Written Warning issued to [Employee Name], Account Executive, in response to a pattern of attendance and communication conduct that is inconsistent with company policy. On three occasions between February 3rd and February 21st, the employee was absent without prior notification to their manager or HR, in violation of Section 3.2 of the Attendance and Absence Policy which requires a minimum of two hours advance notice for unplanned absences. This Written Warning will be placed on the employee's personnel file and will remain active for 12 months. Required Corrective Actions: (1) All unplanned absences must be communicated to the line manager via phone or message by 9:00am on the day of absence, without exception. (2) The employee must achieve full adherence to the notification requirement for all 12 weeks of the active warning period. (3) A check-in meeting with the manager will occur weekly for the first four weeks to discuss any barriers to attendance. This decision may be appealed in writing to HR within 10 working days of receipt of this letter.
